Data must be fully correct and representative of the action or event.
Complete, Truthful & Free from Errors
For data to be accurate, it should be complete, truthful, and free from errors that are not justified. There must be no question regarding the accuracy of the data.
A common issue is full disclosure of the data itself, especially in places where data must be discussed in a parallel path such as an investigation or complaint record.
In these situations, companies need to be careful that original data is not modified or excluded.
Annotating the Amendments
Editing should not be performed without documenting and annotating the amendments.
For example:
- Use a second person witness check for critical record collection to confirm accuracy of data
- Consider how to capture data electronically and verify its accuracy. Build accuracy checks into the design of the electronic system
- Place controls/verification on manual data entry, for example, temperature results can only be entered within a predefined range of 0-100°C
